MarkBook® section 5-2
Mark/Grade Recording Form

MarkBook prints this recording form on standard paper. A portion of one of these printouts is shown in this sample. Scanning and shrinking for this display have made this image fuzzy but your printouts will be as sharp as your printer permits.

Note that all student names are merged with the cells on the left.Mark/Grade Recording Form This saves time - no more manual printing of names in a standard paper record book. Reprint the form whenever the last copy is full or, keep several forms - one for each category of assessments for instance. Punch these forms to store in a binder.

Use this form to record assessment results and to act as the manual backup of electronic records. Each assessment is named at the top of the page. The date of each assessment and its base mark is recorded in the upper two rows. The titles of these two rows can be changed to meet personal needs - see the recording grid form in section 5-1.

This teacher has used red to identify labs. This is one way to classify entries. Or, keep groups of columns for each category or a separate page/sheet for each category. Given that MarkBook cross-classifies each assessment by date and category, it's not absolutely necessary to identify categories on this form.

Incomplete assessments are blank. MarkBook provides two different treatments for these missing items. Enter these in the computer as "NoMark" which means that the assessment is "forgiven" and will not count for or against the student. Or, enter a "Zero" which will penalize the student for not completing these assessment(s).

This form was set up by clicking on Class List and selecting Print as in section 5-1. Or, select Printed Forms from the main operating screen. Note that the teacher elected to customize top two optional rows for the date and base mark. You also have the option of specifying the spacing of darker lines. The default is every 5 columns as shown in this printout.

Tip: as you record assessment data in the computer, each MarkBook assessment entry is automatically numbered as in section 7-2. To help correlate your computer and paper records, use the bottom row of this form to record MarkBook's Entry Number.
